A painful reach is a symptom, not a diagnosis
Reaching into a cupboard, putting on a coat and lifting equipment are different tasks. Shoulder pain during one of them does not, by itself, identify a rotator-cuff injury or another condition. An assessment helps determine what care or further investigation may be appropriate.
Start with the task that matters most to you. “I cannot comfortably put a plate on the upper shelf” tells your clinician about a practical limitation without requiring you to label the cause.
When to seek urgent medical help
Get urgent medical advice for sudden severe shoulder pain, inability to move your arm, marked swelling or a change in shape, persistent numbness, or fever with feeling unwell. Pain following an accident or fall also needs prompt medical advice. Do not delay urgent care while waiting for a routine physiotherapy appointment.
Prepare a short reaching and lifting history
Note when the problem started and whether it followed a particular event or developed gradually. Describe the direction of reach, what you are carrying and whether the task has become more difficult. Tell the clinician about symptoms at rest or at night as well.
If work is affected, bring a simple description of your actual duties: shelf height, repeated movements, loads and available breaks. For recreation, describe the activity rather than only saying “I exercise.” You do not need to repeatedly test a painful movement to gather this information.
What should you ask at the assessment?
Ask what the findings mean and whether you need another medical assessment or imaging. Do not assume a scan is either necessary or unnecessary before discussing your history and examination.
Ask which everyday movements you can continue, which may need adjusting, and how to make those adjustments. If exercises are suggested, request clear instructions about technique, frequency and what response should prompt advice. The NHS advises against inventing strenuous exercises or using heavy gym equipment to treat shoulder pain yourself.
Turn a general goal into a useful one
“Get my shoulder better” is understandable, but a concrete task makes the discussion clearer. Consider “reach the kitchen shelf comfortably” or “manage the lifting required at work.” Ask how progress toward that goal will be checked.
Bring previous reports, surgical instructions or advice you already have. Tell the clinician about strategies you have tried and whether they helped. Recovery and the number of appointments vary; a general article cannot predict either for you.
